AbilisHealth home health care clinical teams support patients in remaining independent at home. Our skilled nursing and therapy teams provide care to patients facing chronic conditions such as Diabetes, CHF and other cardiac conditions, COPD and other pulmonary conditions, as well as treating other short-term conditions such as wound care, IV infusion, infectious disease.

Skilled Nursing Services

Our nurses coordinate all services ordered by a doctor or nurse practitioner to ensure the best care and outcomes specific to each patient.

Care may include:

    • Physical assessments with routine checks of vital signs
    • Education about taking care of and managing illness or injury
    • Lab work to monitor conditions like diabetes
    • Feeding tube care and education
    • Insertion and care of catheters and education to prevent any issues with catheter utilization
    • Care to promote wound healing
    • Education and management of ostomy equipment 
    • Education and care for, but not limited to, diabetes, high blood pressure, difficulty breathing, and heart failure
    • Pain management
    • Comprehensive medication management and education, including injections and IV therapy
    • Diet education to support lowering issues with high blood pressure
    • Ventilator monitoring and education

    Around the clock care is available with certain insurance coverage or by private pay.

Physical Therapy

Our physical therapists work to return our patients to their previous level of function following an illness or injury. When the previous level of function is not possible, our physical therapy team will work with patients and their caregivers to establish techniques for maintaining the highest level of function possible. Maintaining strength, endurance, and flexibility are key factors in recovery.

Care may include:

  • Therapeutic exercises including a home exercise program
  • Education on transferring or moving from one object to another. For example, moving from a toilet to walker or from a wheelchair to a bed
  • Activities to improve stability while walking to reduce the risk of falling
  • Therapy with special devices or equipment such as ultrasound, electro-therapy (to reduce pain or improve muscle function), walkers, and wheelchairs
  • Education and training on proper use of a prosthetic device
  • Muscle strengthening and training

Speech Therapy

The goal of speech therapy is to improve all aspects of speech, communication, and swallowing disorders by using the latest techniques.

Care may include: 

  • Treatment of voice disorders
  • Development of patient skills to address problems with speech or difficulty forming words
  • New ways of expressing yourself or understanding language
  • Treatment of problems with swallowing
  • Assistance with diet modification to improve swallowing
  • Education on new methods to improve hearing loss
  • Development of visual and hearing aid programs
  • Development of memory tasks programs to maintain thought processes

Occupational Therapy

Our occupational therapists recognize that returning to independent living, work and leisure are all important steps to recovery. Occupational therapists provide individualized treatment and education on adaptive skills to help obtain the highest quality of life.

Care may include: 

  •  Training on self-care and daily activities like bathing, dressing, and cooking

  • Training on instrumental activities like sleep habits, home management, and social interactions

  • Muscle strengthening, training, and re-education

  • Treatment related to movement control problems

  • Sensory treatment to improve function

  • Use of equipment like splints and adaptive devices to improve function.
