Home Health Care

Home health care is skilled care provided by credentialed healthcare professionals, including nurses, therapists, and social workers. Patients benefit by being able to stay in their homes instead of a hospital or assisted living facility.

Care at home often makes it possible for patients to leave the hospital sooner after an acute episode. Home health care can also help patients stay independent or even prevent acute episodes from happening in the first place by keeping patients healthier through regular visits by healthcare professionals.

Home health care includes skilled nursing — such as IV therapy, wound care, pain management, and diet instructions — as well as physical, speech, and occupational therapy. Home health can be covered by Medicare if the patient:


  • Is under the care of a physician
  • Requires skilled care services (from an RN, LPN, PT, PTA)
  • Needs intermittent care (versus long-term, continuous care)
  • Is homebound
